Survivor or already gone?

Any good advice to safe this little guys life or is it just too late? The soil is dry. It gets water once every three to four weeks. It's still attached to the soil but looks so sad. The dry base makes me think it's already to late. It's facing south and gets direct sunlight. Should I consider a new pot or is that too much stress?
