How to Answer on Salary Amount?

Hi, I'm working though an application form right now and on the section on info of my last job it asks what its salary was. I've done the application and interview song and dance just fine but I don't have a huge amount of experience and don't know how those answers are usually formatted. Do you write in your pay per hour? The total you made? How much you'd hypothetically make per year (this was a 6 month contract with weekly payslips so I wasn't there that long)?

Google hasn't been much help and just keeps pointing to salary expectations, and the application closes in a couple of days, so I'd appreciate any advice. Thanks!
